Deadline for All Permission Slips (Phase 3) is June 1, 2023
All permission slips and questions need to be completed in apply.yleana.org before June 1, 2023. We refer to these permission slips and questions as phase 3. Every student needs to complete all of Phase 3 before June 1, 2023. If you do not complete all of Phase 3, you may no longer be eligible to come to our summer program. All of your forms and questions must be submitted through apply.yleana.org. We do not accept any hardcopies of the forms.
To access phase 3:
log into apply.yleana.org with the same email/password you used when you first completed your application

What You Need to Submit by June 1
SAT admission ticket
All students must register for the August 26, 2023 SAT through College Board. Yleana is unable to register students. Once you register, upload a picture or copy of your admission ticket to the upload box in Phase 3.
We know that the registration for the August SAT isn’t open yet. If you aren’t able to register for the August SAT before June 1, you can submit Phase 3 without it.
If you aren’t able to register by June 1, you should regularly check the College Board and register as early as you can. We predict that sites will fill up extremely quickly. We will follow up with you to get your admission ticket, but it is your responsibility to register for the test on your own.
If you need a fee waiver, get it from your school counselor. Yleana is unable to provide fee waivers.
Roommate questionnaire
Fill out the questions so we can place you with a compatible roommate. We traditionally separate students by gender (one dorm for female students and one dorm for male students) and place students with roommates who are not from the same school/partner organization so you get to know other students. You may have one or two roommates depending on the exact dorm we are in. We strive to make all students comfortable; if you need housing accommodations of any kind, there is a section of the form where you can let us know.
Yleana permission slips
Student form: Enter your own email, then complete the form through HelloSign, download the completed form, and upload it into the upload box in Phase 3.
Parent/guardian form: Enter your primary caregiver’s email. The form will be emailed directly to them via HelloSign. They need to complete the form, then send you the signed form. You should download the signed form and upload it into the upload box in Phase 3.
Medical form
The medical form needs to be printed and completed by a doctor if you have not had a physical since August 7, 2022. Then you can take a picture of it and upload it to the upload box in Phase 3.
NOTE: If your last physical was after August 7, 2022, you can upload any documentation you received from that visit, including your immunization (vaccine) records.
COVID vaccination card
All staff and students are required to be fully vaccinated (including boosters) before coming to the summer program. Upload a picture of the front and back of your COVID vaccination card to the upload box in Phase 3.
Insurance card
Upload a picture of the front and back of your health insurance card to the upload box in Phase 3.
Contact us if you have questions about anything that’s required for Phase 3.

Summer Program 2023 COVID Policies
The health and safety of our students and staff is the most important thing to us. The Yleana Leadership Foundation is implementing the following COVID policies for this year. Yleana Leadership Foundation reserves the right to change these policies as we see fit or as required by our campus partners. (Updated 5/18/23)
All students and staff are required to be fully vaccinated, including any applicable boosters.
We will verify that all staff/students have a valid vaccine card.
We recommend that all students test themselves 24 hours before getting on the bus. Students who test positive should not get on the bus.
Yleana will not send COVID tests to students before camp
We recommend that all students and chaperones wear masks and socially distance, as much as possible, on the buses to and from our summer program.
Students should plan to sit with at least one empty seat between them and the next closest student, if possible based on the size of the bus.
We will have masks and rapid tests available at camp.
Students who experience mild symptoms consistent with COVID will be tested with a rapid test.
Students who test positive will unfortunately be unable to continue staying at our summer program. In the case of a positive test, we will alert the campus’ medical staff about the positive test, alert the student’s emergency contact, and move the student to an isolated room in the dorm. Students’ emergency contacts should be prepared to drive to Colgate or Mount Holyoke to pick up their student in case they test positive.
Students who test negative will be required to wear a mask, test every other day, distance from the rest of the students/staff until their symptoms resolve and/or they have 2 negative tests.
We will follow all additional rules and regulations required by Colgate/Mount Holyoke to help limit the spread of COVID on campus.
